As a 100-percent customer-owned cooperative, Sunrise Cooperative would be nothing without the support and involvement of our customer-owners. We have gathered all the forms and information needed to become a customer or member in one place to help streamline your process. Please feel free to contact Sunrise customer service if assistance is needed. 
 

Membership Applications

There are two types of memberships within Sunrise Cooperative: Common Stock and A Preferred Stock. Both of these membership types have multiple categories as shown below. Common Stock membership costs $100 and you are a voting member through your business or as an individual. A Preferred Stock membership costs $50 and this is a non-voting membership setup as a consumer, business or farm.

To become a member, you must submit the following:

  • Common Stock ($100) or A Preferred Stock ($50) membership application
  • Credit Application to establish an account, if an account is not currently established
  • W-9 Form - allows us to pay patronage without deducting federal tax.

W-9 Form


Completing the W-9 Form allows us to pay patronage without deducting federal tax. If you select the Digital W-9, be sure to save it as a PDF, then email it to creditall@sunriseco-op.com.

Input Finance

Input Financing

The Sunrise Input Financing Program allows you to finance your fall, spring and summer crop input purchases. The program also allows you to finance your agronomy prepay purchases from Sunrise Cooperative. Eligible charges are to be made between September 1st and August 31st of the present year. To qualify for the program you must apply for a minimum of $10,000 to be financed.

Direct Debit Payment Plan

By participating in our Direct Debit ACH payments, the current month’s due invoices would be directly deducted on the 25th of that month automatically from the bank account indicated on the enrollment form.  You would then know your bill is paid on time for the end of month deadline. In order to receive payment on the 25th of the month, the Direct Debit ACH form must be submitted by the 20th of that month.

To update or cancel a Direct Debit ACH payment please contact the credit department at, 419-332-6468 ext 205 & ext 229, or complete a new Direct Debit ACH form.

Grain Payment ACH Deposit Form

Our Grain Payment ACH Deposit Form allows you to receive automatic deposits of your grain settlement payments. Beginning July 1, 2022 all Grain payments will be made via ACH. 

  • All Grain payments will be made via ACH.
  • Grain Payments will only be initiated on Tuesdays and Fridays.
  • Grain Payments will only be made upon your request.
